DS-157 Supplemental Nonimmigrant Visa Application

The DS-157 is a supplemental visa application for non-immigrant visas in which the petitioner is requested to provide information that will assist the relevant authorities in the conduct of their security background investigations. This form is presented to the consulate when applying for an NIV. In most cases, this information is requested of males between the ages of 16-45.
